Register with us as a New Patient

You can register with The Symons Medical Centre using the online forms below. If you have any queries regarding registration, or would prefer to complete your registration in person, then please contact our Reception Team on 01628 626131.

Catchment area

We are currently OPEN for registrations for patients who reside within our inner boundary.

We have a small number of registered patients who reside outside of this area. This is due to historical reasons (e.g. the patient was registered at their address within the boundary but then the boundary subsequently changed with the agreement of NHS England). Due to increasing demand on our services and the need to ensure that we can provide the best possible care to those within our catchment area, we are in the process of routinely writing to patients who reside outside of our boundary to advise them to re-register at a surgery closer to their home address. 

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note: it can take up to 20 working days for your registration to be processed.

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Organ Donation
